Uncontrolled type 1 diabetes mellitus may cause bilateral diabetic snowflake cataract in young adults due to diffusion of glucose into lens and its conversion into sorbitol (by enzyme aldolase reductase) which accumulates in lens fibers and osmotically bring in water into lens leading to swelling of lens fibers and eventually their rupture resulting in development of white, anterior and posterior subcapsular and cortical opacities with vacuoles and water clefts.
